Decisions, open questions and risks across the portfolio

How the Decision Log, On the Table and Risks & Issues bring every project's records together.

Updated 31 October 2026

Decision Log

Every decision on every project, with the reasoning recorded at the time. The figures at the top count the decisions recorded, those Awaiting a decision, how many carry their Rationale recorded, and how many were Approved by a finance body.

Filter by Status (All, Agreed, Approved or Pending) and by Project, typing part of its name. Pending decisions are listed under Awaiting a decision, the rest under Decisions taken. A decision taken with less than 60 characters of reasoning is marked Rationale missing.

On the Table

The questions not yet answered. Choose Show: Open (the default), Decided or All. Each one shows when it is needed by and who owns it, and Overdue: needed by its date once that date has passed.

The button that counts the options, such as Show the 3 options, shows each option's Gain, Give up and Cost, with its verdict. Recommendation on the table is what is being brought to the board. A question with No options recorded is flagged.

Risks & Issues

Every risk is scored likelihood × impact, from 1 to 9:

Likelihood, by impactLowMediumHigh
High369
Medium246
Low123

Filter by Severity, Trend, Project or Owner, and select a column heading to sort. A risk is flagged when it has No owner, a Thin mitigation (under 40 characters) or No next action.

The Exposure matrix plots the risks shown: high exposure is a score of 6 to 9, monitor is 3 to 4, low is 1 to 2. Issues: already happened lists things that have gone wrong and are being worked.
