Record milestones, decisions, risks and updates

Add and change the records each project carries, and the rules each one follows.

Updated 31 October 2026

Where to record things

  • On the project's edit page, Contents lists each kind of record with Manage and + Add.
  • On the project's board page, each list has + Add and Manage. Saving takes you back to that tab.

The edit page's Completeness panel shows what the project still lacks. Milestones, decisions, risks, benefits, stakeholders and updates are required before a project goes to the board.

The records and their rules

  • Milestones: a Baseline date (the date committed to) and a Forecast date (the date now expected). State is done, due or wait, shown on the board as Complete, Due and Planned. Tick This is a stage gate for gate milestones. A forecast later than its baseline needs a Note saying why.
  • Decisions: Status is Agreed, Approved or Pending. For one not yet taken, type pending in Date taken. Any decision that is not pending needs at least 60 characters in Why — the rationale. Use Relates to for the risk, question or gate it settles.
  • Risks: choose Severity, Likelihood and Impact. The Score (calculated) is likelihood × impact, worked out on the server; it cannot be typed. Name a person as Owner, write a Mitigation of at least 40 characters, and add a Next action.
  • Issues: things that have already happened. Say What is happening about it, in at least 30 characters.
  • Benefits: How it is measured, with its Baseline, Target and Confidence.
  • Stakeholders: who they are, their interest, their Stance and How we are engaging them.
  • Updates: What changed, at least 30 characters, written for someone outside the project. A project with no update for 45 days is flagged.
  • Documents: listed by title and detail. You cannot upload a file.
  • In scope: each workstream and what it covers.
  • Headline figures: shown across the top of the project's page. Five at most are shown.

Questions and their options have a guide of their own: Put a question on the table with its options.

References and suggestions

A new decision, question, risk or issue gets the next free reference, such as R-33. Decision, question and risk references must be unique across the portfolio.

Fields for people, deciding bodies, areas and references suggest matches as you type. They are suggestions only: you can type anything.

Change or remove a record

Choose Edit in the list, change it, and choose Save changes. To take one away choose Remove; the dialog warns if other records mention it. Choose Keep it or Remove.

Every change is a draft and is written to the change log. The board sees it once it is published.
